Benchmark Human Services is seeking a Licensed Practical Nurse (LPN) to provide one-on-one skilled nursing care in the homes of infants, children, and young adults in Crown Point and Schererville. This role supports families and caregivers, enabling independent living with the smallest possible hospital-like settings.

Position offers full-time and part-time opportunities with flexible scheduling and no mandatory 12-hour shifts, weekends, or holidays.

Qualifications

• Valid Indiana Nursing License (LPN) required.
• Willing to travel to nearby clients in need of assistance.
• Trach, G-tube, and/or Vent experience preferred.

Responsibilities

• Provides professional nursing care as defined in the nurse practice act under the direction of a RN.
• Communicates status changes to the RN Clinical Care Manager or designated RN.
• Follows the Plan of Care and Nursing Care Plans established by the physician and RN.
• Recognizes and reports changes in client condition to the RN Clinical Care Manager and physician.

HomePointe HealthCare is currently hiring for a full-time or part-time Licensed Practical Nurse (LPN) position in Crown Point and Schererville. Join our team and make a difference in the life of a child while providing one-on-one skilled nursing care in the home.

Schedule:

• Crown Point: Full-time Evenings into some nights
• Crown Point: Part-time Days
• Schererville: 12 shifts with weekend availability

Responsibilities:

• Provides professional nursing care as defined in the nurse practice act and under the direction of a RN.
• Communicates appropriate status/condition changes to the RN Clinical Care Manager or designated RN.
• Provides skilled nursing care for clients as directed by the RN Clinical Care Manager.
• Follows the Plan of Care and Nursing Care Plans established by the physician and RN.
• Recognizes and reports changes in client condition to the RN Clinical Care Manager and physician, as directed.
• Communicates information on a timely basis, consistent with Agency policy, and urgency of the situation. Communicates verbally, as needed, and through complete and accurate documentation in the clinical record.
• Take verbal orders by a physician or qualified medical personnel and record. This will be followed by a co-signature from the Clinical Care Manager or designated RN scheduling
• Other duties as assigned.

Requirements:

• Valid Indiana Nursing License (LPN)
• Valid Driver's License
• Willing to Travel to Nearby Clients in Need of Assistance
• Trach, G-tube, and/or Vent Experience Preferred
