Benchmark Human Services is seeking a full-time and part-time Licensed Practical Nurse (LPN) for positions in LaPorte and Michigan City. The role involves providing one-on-one skilled nursing care for children in the home environment. Candidates will have the autonomy to work independently while ensuring the highest standard of care.

Qualifications

• Valid Indiana Nursing License (LPN) is required.
• Valid Driver's License for client travel.
• Experience with Trach and G-tube preferred.

Responsibilities

• Follows established Plan of Care and Nursing Care Plans.
• Documents client interactions in the clinical record.

HomePointe HealthCare is currently hiring for a full‑time and a part‑time Licensed Practical Nurse (LPN) position in LaPorte and Michigan City. Join our team and make a difference in the life of a child while providing one‑on‑one skilled nursing care in the home.

Schedule

• LaPorte: Full‑time Day shift providing care for a pediatric client with a G‑tube button
• Michigan City: Part‑time Friday 9am‑7pm & Saturday 9am‑6pm providing care for a pediatric client with a trach

Responsibilities

• Provides professional nursing care as defined in the nurse practice act and under the direction of a RN.
• Communicates appropriate status/condition changes to the RN Clinical Care Manager or designated RN.
• Provides skilled nursing care for clients as directed by the RN Clinical Care Manager.
• Follows the Plan of Care and Nursing Care Plans established by the physician and RN.
• Recognizes and reports changes in client condition to the RN Clinical Care Manager and physician, as directed.
• Communicates information on a timely basis, consistent with Agency policy, and urgency of the situation. Communicates verbally, as needed, and through complete and accurate documentation in the clinical record.
• Take verbal orders by a physician or qualified medical personnel and record. This will be followed by a co‑signature from the Clinical Care Manager or designated RN scheduling.
• Other duties as assigned.

Requirements

• Valid Indiana Nursing License (LPN)
• Valid Driver's License
• Must Pass a Thorough Background Check
• Willing to Travel to Nearby Clients in Need of Assistance
• Trach, G‑tube, and/or Vent Experience Preferred
