Under supervision, you will coordinate additional patient services to ensure high-quality outcomes in the service area. The role requires LA LPN licensure, CPR, reliable transportation, and 1+ year of LPN experience.

Qualifications

• Current LPN licensure in LA without restrictions.
• Current CPR certification.
• Valid driver's license and access to reliable transportation.

Responsibilities

• Provides high quality clinical nursing in patients' homes per physician plan of care.
• Documents visits thoroughly in the electronic medical record and adheres to regulations.
• Coordinates care and communicates with patient, family, physician, and care team.
