Job summary

Optum seeks a Home Health Licensed Practical Nurse (LPN) for Minden LA PCS, part of LHC Group. Providing direct nursing care in patients' homes under supervision, you will document visits and coordinate care to ensure high-quality outcomes in the assigned service area.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
• LA: Current LPN Licensure in LA without restrictions; 1+ years of experience as an LPN.

Responsibilities

• Provides high quality clinical services within the scope of practice and plan of care.
• Documents patient visits thoroughly in the electronic medical record.
• Participates in care coordination, communicates with patient, family, physician and care team.
