Responsibilities

• Provides high quality clinical services within the scope of practice in the home setting, following the plan of care and infection control procedures.
• Documents patient visits thoroughly in the electronic medical record.
• Coordinates care and communicates needs to patients, families, physicians, and care coordinators.
