Job summary

Optum, through Harahan LA PCS and the LHC Group, seeks a Home and Community Based Services Licensed Practical Nurse to deliver direct nursing care in patients’ homes within the service area. You will assess, follow physician orders, document visits, and collaborate with the care team to ensure high-quality patient care.

Under supervision, you will help coordinate services, complete EMR documentation, participate in care planning, and adapt to diverse home environments.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current CPR certification.
• Current driver's license and access to a vehicle or reliable public transportation.

Responsibilities

• Provide high-quality clinical nursing services per the plan of care and infection control procedures.
• Thoroughly document patient visits in the EMR and agency systems.
• Participate in care coordination, report patient needs and changes, and communicate with the care team.
