Team Select Home Care is seeking a Home Health Aide to provide in-home care for medically fragile children and adults under an RN's direction. You will perform personal care, assist with daily living activities, and help patients remain independent in their own homes.

The role emphasizes safety, confidentiality, and compassionate communication. Required training includes 86 hours, CPR certification and AHCA screening, with opportunities for ongoing education and career development.

• Clear reading, writing, and communication skills.

Responsibilities

• Provide in-home personal care as ordered in the care plan.
• Assist with activities of daily living and medications as allowed.
• Document visits timely and report changes to RN.
• Maintain confidentiality and professional conduct at all times.
• Attend required trainings and team meetings.
