Job summary

Phoenix Children's is seeking an experienced Clinical Nurse for its Outpatient Hemodialysis clinic in Phoenix. The clinic operates 6 AM - 5 PM and serves four patients at a time. Must have at least 1 year of dialysis experience or 2 years RN inpatient experience, or 1 year RN with HD CCHT.

• Continually evaluates patient responses to care and documents progress for clinical teams.

Responsibilities

• Completes nursing assessment of patient conditions and factors to determine initial treatment plan adherence.
• Continuously evaluates patient responses to care, teaching, and discharge plans, and documents progress.
• Contributes to team by performing activities within regulatory standards and documentation.
• Provides orientation to the unit, hospital systems, and processes to patients and families.
• Supervises care activities of assigned team members and delegates tasks appropriately.
• Works with care teams to plan and coordinate patient health status and care rounds.
