Job summary

Hackensack Meridian Health is seeking a Clinical Coordinator (Registered Nurse) to collaborate with the Clinical Program Manager and subspecialty teams. You will coordinate care for complex patients, provide education to families and staff, and ensure high-quality nursing care across subspecialty programs.

You will act as a liaison among families, physicians, and departments, document care plans in the EMR, and participate in care coordination, testing, and treatment planning to improve patient

Qualifications

• RN license in NJ required.
• BSN required.
• 3–5 years of acute/primary care nursing experience.

Responsibilities

• Maintains a communication system with providers and multidisciplinary team.
• Assess patient needs during visits and via phone, collaborate on evidence-based solutions.
• Document all interventions and care plans in EMR.
• Provide nursing assessments and direct patient care.
• Coordinate subspecialty clinics, scheduling, and clinical direction to staff.
• Serve as clinical resource and develop education and operation guidelines.
• Participate in evidence-based guidelines and outcome monitoring.
• Contribute to process improvements to enhance patient outcomes.
