Qualifications

• Bachelor's of Science in Nursing.
• Minimum of 3-5 years acute/primary care for specialty and population specific nursing experience.
• Proficient computer skills (MS Office and/or Google Suite).
• Excellent written and verbal communication skills.

Responsibilities

• Identify, support and develop programs to meet the needs of complex patients and families.
• Provide professional nursing care and support to patients/families with subspecialty needs.
• Serve as a liaison between the family and the subspecialty team to assist in care coordination.
• Provide disease-specific education for patients, families and staff.
