Job summary

The Children's Hospital of Richmond Gastroenterology Clinic is seeking a licensed clinical social worker (MSW) to join our interdisciplinary team. You will provide direct patient and family support, complete psychosocial assessments, and coordinate services across care teams to address mental health, safety, and family needs.

Responsibilities include developing care plans, delivering interventions, and coordinating with schools and community resources to ensure comprehensive discharge planning.

Qualifications

• Licensed Clinical Social Worker preferred in Virginia.

Responsibilities

• Provide direct patient care to patients and families and coordinate team-delivered services.
• Complete psychosocial assessments and develop treatment plans aligned with the health care plan.
• Deliver psychosocial interventions and educate patients, family members, staff and students.
• Coordinate discharge planning and community referrals; support schools and child services where needed.
• Engage in quality improvement activities and participate in care conferences.
