The role involves coordinating with schools, CPS, and community resources, ensuring seamless care for families.

The ideal candidate has an MSW, LCSW preferred, and at least three years of clinical social work experience, preferably in acute care

Formación

• Master of Social Work required from an CSWE-accredited program.
• Licensed Clinical Social Worker preferred; three years of clinical social work experience required, including pediatric or specialty practice.
• Experience coordinating care with schools, child protective services, and community resources.

• Provide psychosocial assessments and develop treatment plans aligned with the overall health care plan.
• Deliver psychosocial interventions and supportive counseling to patients and families.
