Job summary

Boston Children's Hospital seeks a compassionate Clinical Social Worker to join our outpatient GI teams, including the Center for Inflammatory Bowel Disease and Celiac Center. This is a 36-hour weekly, hybrid role with on-site Fridays in Boston and alternating Mondays at the Needham satellite, remote on other Mondays.

Eligible for full benefits and a $4,500 sign-on bonus. The role focuses on psychosocial care, care coordination, family support, and school collaboration for Crohn's, Ulcerative

Qualifications

• Master's degree in social work required.
• Previous medical social work experience highly preferred.
• Excellent verbal and written communication and organizational skills.
• Fast-paced, dynamic environment requires flexibility.
• Strong clinical experience in pediatric settings; GI or chronic illness a plus.

Responsibilities

• Conduct psychosocial screenings and comprehensive assessments to guide care planning.
• Provide brief individual and family support focused on adjustment to chronic illness and family system support.
• Offer longitudinal care coordination and case management, facilitating access to community and hospital-based support.
• Help families navigate education systems and collaborate with schools for accommodations.
• Collaborate with physicians, nurses, dietitians, psychologists, and other teams.
• Foster open communication and teamwork within multidisciplinary teams.
• Deliver crisis intervention, safety planning and resource mobilization as needed.
• Address child protection concerns in collaboration with agencies.
• Contribute to program development and quality improvement efforts.
