The ideal candidate will have a pharmacy degree from an ACPE-accredited program, Texas licensure, and pediatric or specialty pharmacy experience. Clinical program development is a plus.

Qualifications

• Graduate from an ACPE-accredited school of pharmacy.
• Texas Pharmacist License or eligible for Texas licensure.
• Pharmacist and pharmacy preceptor licensure within 90 days of employment.
• Basic Life Support for Healthcare Providers certification.
• Pediatric Advanced Life Support certification within 60 days of hire (critical care areas).
• PGY1 residency accredited by ASHP or 3 years commensurate pharmacist experience in a comparable health-system setting is strongly preferred.
• Previous pediatric experience in a hospital setting is strongly preferred.
• Previous specialty pharmacy experience is preferred, but not required.
• Formal diabetes education (CDCES or BC-ADM) preferred but not required.

Responsibilities

• Provide direct patient care and coordinate transition of care for patients with chronic health conditions.
• Conduct comprehensive medication management for endocrinology-related conditions.
• Provide medication counseling to patients and caregivers.
• Monitor therapy and adjust regimens as needed.
• Review patient charts to identify and manage drug-related problems.
• Provide consultative pharmacotherapy services to other clinicians.
• Identify and implement improvements in the medication-use system to maximize safety and outcomes.
• Develop training opportunities for students and staff.
