Job summary

Cook Children’s Health Care System is seeking an Ambulatory Care Clinical Pharmacist in Fort Worth to manage safe and effective pediatric medication therapy in an ambulatory setting, with a primary focus in Endocrinology. The role emphasizes direct patient care, collaboration with clinicians, and development of pharmaceutical plans for complex pediatric patients.

Eligible candidates will have an ACPE-accredited degree and Texas licensure; BLS and PALS are required with potential for PGY1

Qualifications

• Graduate from an ACPE-accredited school of pharmacy.
• Texas Pharmacist License or eligible for Texas Licensure.
• Pharmacist licensure must be obtained within 90 days of employment.
• Current Basic Life Support (BLS) certification.
• Pediatric Advanced Life Support (PALS) certification within 60 days of hire.
• PGY1 residency accredited by ASHP or 3 years of pharmacist experience in a comparable health-system setting (preferred).
• Previous pediatric experience in a hospital setting (preferred).
• Previous specialty pharmacy experience (preferred).
• Formal diabetes education (CDCES or BC-ADM) preferred.

Responsibilities

• Provide direct patient care and coordinate transition of care for patients with chronic health conditions.
• Conduct comprehensive medication management for endocrinology-related conditions; provide counseling and monitor response to therapy.
• Review patient charts to identify and manage drug-related problems, dosing, interactions, and access issues.
• Provide consultative pharmacotherapy services to other healthcare professionals.
• Identify and implement improvements in the medication-use system to enhance patient safety and outcomes.
• Develop training and education opportunities for students, residents, or staff.
