Job summary

Cook Children's Medical Center in Fort Worth, TX seeks an Ambulatory Care Clinical Pharmacist focused on pediatric endocrinology to provide safe medication management in an ambulatory setting. You will collaborate with clinicians to develop treatment plans and ensure high-quality pharmaceutical care for children with chronic conditions.

Responsibilities include direct patient care, counseling, and ongoing therapy optimization within a 40-hour work week, Monday through Friday, usually 8:30am–5pm.

Qualifications

• Graduate from an ACPE-accredited school of pharmacy is required.
• Texas Pharmacist License or eligible for Texas licensure.
• Licensure as pharmacist and as a preceptor within 90 days of employment.
• BLS for Health Care Providers and Pediatric Advanced Life Support within stated timelines.
• PGY1 residency or 3 years’ pharmacist experience in a comparable health-system setting preferred.
• Pediatric hospital experience preferred; specialty pharmacy and diabetes education are plus.

Responsibilities

• Provide direct patient care and manage medication therapy for pediatric endocrinology patients.
• Counsel patients and caregivers on medications and therapy plans.
• Conduct comprehensive medication reviews and monitor therapy outcomes.
• Collaborate with clinic staff to ensure safe, effective pharmacotherapy.
• Identify process improvements to enhance the medication-use system and patient safety.
• Participate in training opportunities for students, residents, or staff.
