Job summary

Cook Children’s in Prosper, TX seeks a Clinical Ambulatory Pharmacist to join the pediatric ambulatory care team. You will educate patients and caregivers, collaborate with multidisciplinary providers, and support medication access and specialty coordination across multiple clinics.

This role focuses on optimizing medication therapy, patient education, and care coordination within ambulatory pediatric clinics, with a schedule Monday-Friday 8a-5p CST.

Qualifications

• Graduate from an ACPE-accredited school of pharmacy.
• PGY1 residency or 2+ years commensurate health-system experience.
• Texas Pharmacist License or eligibility to practice in Texas.
• BLS for Healthcare Providers required; PALS within 60 days for critical care areas.

Responsibilities

• Provide comprehensive medication education and counseling to pediatric patients and caregivers.
• Collaborate with multidisciplinary providers to optimize medication therapy and improve patient outcomes.
• Support medication access initiatives including prior authorizations and appeals, and financial assistance programs.
• Coordinate with outpatient and specialty pharmacy teams to facilitate procurement, adherence, and therapy management.
• Monitor and manage patients on specialty therapies with appropriate follow-up.
• Serve as a clinical resource for providers on medication selection, dosing, monitoring, and alternatives.
• Participate in quality improvement initiatives to enhance ambulatory clinic care.
