Job summary

The Direct Care Nurse at ST MARYS HOME in Virginia provides specialized nursing care to individuals with medical and physical disabilities, supporting health plans and family involvement.

Working collaboratively with physicians, caregivers, and the broader care team, the nurse delivers compassionate, person-centered care, conducts assessments, administers medications, and documents outcomes in the EHR to ensure high-quality support for residents.

Qualifications

• Must be a graduate of an accredited nursing school.
• Must hold a valid, active Virginia LPN or RN license.
• Preferably one year of experience in pediatric or medical/surgical nursing.
• Experience with complex medical conditions and equipment (e.g., tracheostomies) is desirable.
• Current CPR certification.

Responsibilities

• Conduct daily health assessments of individuals to monitor conditions.
• Monitor health status, vital signs, and treatment effectiveness.
• Administer medications and treatments per orders; document outcomes.
• Document nursing interventions in the EHR promptly and accurately.
• Perform wound care and tracheostomy care as needed.
• Respond to emergencies and implement protocols safely.
• Maintain nursing supplies and monitor inventory.
• Adhere to policies, infection control, and confidentiality.
• Engage in continuing education and professional development.
• Provide supervision and preceptorship to new staff.
• Develop and review individualized care plans with the team.
• Coordinate care with physicians, therapists, and families.
• Advocate for patient rights and provide emotional support.
• Communicate health statuses and progress to families and staff.
