Job summary

Saintmaryshome is seeking a Direct Care Nurse to provide specialized nursing care to individuals with medical and physical disabilities in a facility setting. The role requires patient-centered care aligned with each individual’s health plan and collaboration with a multidisciplinary team including families and healthcare professionals.

You will perform daily assessments, monitor changes in health status, administer medications and treatments, document in the EHR, and support emergency response.

Qualifications

• Must be a graduate of an accredited nursing school.
• Must hold a valid, active Virginia LPN or RN license.
• Preferably one year of experience in pediatric or medical/surgical nursing.
• Experience in care for individuals with complex medical conditions is highly desirable.
• Current CPR certification may be required.

Responsibilities

• Assess individuals daily to monitor health conditions and detect changes.
• Monitor health conditions and review care plans for updates.
• Administer prescribed medications, tube feedings, and treatments per orders.
• Document nursing interventions and condition changes in the EHR.
• Perform procedures such as wound care and tracheostomy care as needed.
• Respond promptly to emergencies and follow safety protocols.
• Maintain nursing supplies inventory and track expirations.
• Adhere to facility policies, infection control, and confidentiality.
• Engage in continuing education and training; provide leadership as needed.
• Collaborate with physicians, therapists, and families to coordinate care.
