Job summary

Advocate Health is seeking a registered nurse to provide diabetes self‑management education to patients and their families. The role includes teaching, device instruction (glucose meters, insulin pumps, CGMS), documentation, and collaboration with the multidisciplinary team.

The position is full-time (40 hours/week), Monday through Friday, 8am–5pm, with base pay in the range shown and a comprehensive benefits program. Eligible for licensure in Illinois and CPR certified.

Qualifications

• Associates in Nursing and 2–5 years clinical experience.
• RN licensure in IL or eligibility.
• CPR certification required.

Responsibilities

• Provide diabetes self-management training and education to patients and families.
• Develop and document individualized teaching plans and follow-up based on assessment.
• Collaborate with patients and families to set learning and behavior goals.
• Educate on glucose monitoring, insulin starts, CGMS, and device use.
• Coordinate with physicians and the health-care team for care continuity.
• Bill third-party payers for diabetes education using proper coding guidelines.
• Support quality clinical care as a registered nurse and ensure documentation quality.
