Qualifications

• Bachelor’s degree in nursing required; willing to obtain within three years.
• Two years of clinical nursing experience required with leadership skills.
• Current RN licensure in state(s) of practice; multi-state licensure if applicable.
• Must obtain CDCES certification within three years when eligible.

Responsibilities

• Educates and collaborates with patients on diabetes self-care.
• Develops diabetes self-management and support plans.
• Educates patients/family on diet, activity, monitoring, and insulin management.
• Coordinates care with endocrine team; supports program quality and documentation.
