Job summary

Advocate Health's Merrionette Park Endocrinology team is seeking an Ambulatory RN/ RD to educate and manage the outpatient diabetes population. The role coordinates education for patients, families, and staff, in a high-quality, patient-centered setting.

The successful candidate will hold a RD (Bachelor's) or RN license, with CDCES certification and insulin pump experience, and will work a 40-hour week in Merrionette Park with occasional travel to Orland Park.

Qualifications

• Bachelor's degree in Nutrition (RD) OR RN license issued by the state.
• 2-3 years nutrition experience with exposure to diabetes patients OR 3-5 years in clinical specialty.
• CDCES certification
• Experience with insulin pump therapy
• Knowledge of child growth & development, adolescent behavior, and signs of normal aging

Responsibilities

• Schedule: Monday – Friday: 8a-4:30p (May change based on clinic/patient needs) Position is based in Merrionette Park with occasional travel to Orland Park.
• As a skilled and compassionate Diabetes Educator, you will play a pivotal role in providing organizing, educating and scheduling patients for the Outpatient Diabetes Self-Management Program.
• Participating in instructing outpatient groups and one-on-one sessions.
• Serve as a resource/clinical expert for identified diabetes inpatients.
• Reinforces education to patients / families as requested.
• Provides hospital staff with adequate supplies of Diabetes educational materials.
• Plan and/or participate in any diabetes-related activities offered to the community, e.g. glucose monitoring, screening, lectures.
• Using evidence-based practice, clinical decision making, compassion, and skills communication while leading efforts to create the safest patient environment and the best patient experience across the continuum.
