Job summary

Sanford Health is seeking a Diabetes Educator for the North Peds Clinic A in Bismarck, ND. The role involves educating patients, developing care plans, and monitoring progress to improve diabetes management.

The position requires a BSN, RN licensure, and a CDE or near-term certification, with BLS. Every 7th weekend call rotation at the Medical Center is included, and full-time hours are expected.

Qualifications

• Bachelor's degree in nursing; willing to obtain within three years.
• Current RN license in state(s) of practice and/or multi-state licensure as required.
• Certification as a Certified Diabetes Educator or obtain within three years when eligible.
• Basic Life Support (BLS) certification is required.

Responsibilities

• Educate patients on diabetes self-care, including nutrition, medication, activity, and glucose testing.
• Collect and analyze patient data to monitor health status and personalize care plans.
• Collaborate with healthcare professionals to optimize diabetes management and patient outcomes.
• Provide group education sessions and participate in community outreach.
