Qualifications

• Minimum two years of clinical nursing experience with demonstrated leadership.
• Certified Diabetes Educator certification or plan to obtain within three years.
• Current Basic Life Support (BLS) certification is required.

Responsibilities

• Educate patients on diabetes self-management and create personalized care plans.
• Conduct individual and group education sessions; monitor patient progress.
• Collaborate with physicians and other health professionals to coordinate care.
