Qualifications

• PharmD degree from an accredited school of pharmacy.
• California license in good standing to practice pharmacy.
• 2 years of experience as a pharmacist in a hospital or healthcare setting.

Responsibilities

• Provide safe and effective patient-centered pharmaceutical care and education.
• Review medication profiles and collaborate with clinicians on therapy optimization.
• Educate patients and caregivers on medication use, adverse effects, and adherence.
• Coordinate with the healthcare team to ensure continuity of care and medication access.
