UCSF Health seeks a Pediatric Clinical Staff Pharmacist II to provide pharmaceutical care across inpatient settings, ensuring safe and effective medication use for patients of all ages within the UCSF Health System.

The role collaborates with physicians, nurses, and other clinicians, conducts medication histories, reviews profiles, educates patients, and supports insurance authorizations and drug information services in a fast-paced hospital environment.

Formación

• Must have graduated from an accredited school of pharmacy with a PharmD or equivalent.
• CA licensure in good standing to practice pharmacy.
• Minimum of 2 years of hospital/healthcare pharmacy experience.

Responsabilidades

• Provide pharmaceutical care to patients in the inpatient setting across UCSF Health locations.
• Obtain medication histories and review/assess medication profiles.
• Provide drug information to clinicians and patients and educate patients on medication use.
• Collaborate with physicians, nurses, and other professionals to optimize therapeutic plans and ensure patient safety.

Conocimientos

Pharmacist license (CA)

BLS certification

Hospital pharmacy experience

Educación

PharmD degree

FPGEC Certification
