Job summary

Methodist Le Bonheur Healthcare in Memphis seeks a Registered Nurse to serve as a care coordinator across the patient continuum. You will assess patients’ physical, psychological, and social needs and collaborate with providers to improve outcomes and education for patients and families.

The role requires an RN license in Tennessee, an Associate degree in Nursing, and BLS certification, with preferred BSN. You will operate in a clinical/office setting and participate in ongoing quality care

Qualifications

• Required - Associates Degree in Nursing.
• Preferred - Bachelor’s Degree in Nursing.
• Required - Registered Nurse Tennessee license.
• Required - Basic Life Support (BLS) from American Heart Association.
• Knowledge of patient care, education, screening and follow-up.
• Strong communication, problem solving and conflict resolution abilities.
• Good computer skills and ability to document accurately.

Responsibilities

• Assess the physical, psychological, and social needs of patients and improve their care experience.
• Coordinate communication with providers on behalf of the patient.
• Create a seamless flow through the health system to facilitate care.
• Develop and coordinate education, resources, and activities for patients and families.
• Maintain systems to document, track, and monitor patient data.
• Perform clinical procedures within nursing scope; may perform telephone duties.
