Middlesex County is seeking a dedicated Case Manager to join our Special Child Health Services (SCHS) team in New Jersey. The role coordinates services to promote health, development, independence, and well-being for Children and Youth with Special Health Care Needs (CYSHCN) and their families.

  • Advocate for timely access to services and supports for children and families.
  • Follow up on referrals and coordinate care among providers and agencies.
  • Provide crisis support and assist families in identifying resources and solutions.

Overview

Middlesex County is seeking a dedicated and compassionate Case Manager to join our Special Child Health Services (SCHS) team. This position plays a vital role in supporting Children and Youth with Special Health Care Needs (CYSHCN) and their families by coordinating services that promote optimal health, development, independence, and overall well-being.

The ideal candidate is a licensed Registered Nurse or Social Worker with experience in pediatric case management and a strong commitment to family-centered care. Bilingual proficiency in Spanish is preferred. TheCase Manager will work collaboratively with children, families, health care providers, educators, therapists, community organizations, and other service professionals to identify needs, coordinate resources, develop individualized service plans, and monitor progress toward established goals.

Key Responsibilities:

Needs Assessment

  • Conduct comprehensive assessments with children and families to identify needs across developmental, educational, medical, behavioral, and emotional domains.
  • Identify strengths, priorities, and available family and community resources.
  • Partner with families to establish goals that reflect the child's needs and the family's priorities.

Individual Service Planning

  • Develop, implement, and regularly update Individual Service Plans (ISPs) based on each child's needs, strengths, and family goals.
  • Ensure service plans are coordinated, measurable, and responsive to changes in the child's circumstances.
  • Document assessments, referrals, contacts, progress, and outcomes in accordance with program and regulatory requirements.

Service Coordination

  • Connect families with appropriate medical, developmental, educational, behavioral health, social service, and community resources.
  • Make referrals and assist families in navigating complex service systems.
  • Advocate for children and families to help ensure timely access to appropriate services and supports.
  • Follow up on referrals and coordinate services among multiple providers and agencies.

Crisis Support and Problem Resolution

  • Serve as a responsive resource for families experiencing challenges or service-related concerns.
  • Assist families in identifying appropriate resources, service alternatives, and solutions.
  • Help address complaints and barriers to care through effective communication, advocacy, and problem-solving.

Care Team Collaboration

  • Promote effective communication and coordination among the child's care team, including primary care providers, specialists, educators, therapists, and other professionals.
  • Participate in meetings and case conferences as appropriate.
  • Facilitate information sharing while maintaining confidentiality and professional standards.

Monitoring and Follow-Up

  • Conduct regular follow-up with children, families, and service providers to monitor progress toward established goals.
  • Evaluate the effectiveness of services and make recommendations for changes when needs or circumstances evolve.
  • Update Individual Service Plans as appropriate and ensure continued coordination of care.

Community Engagement

  • Develop and maintain strong working relationships with local and regional organizations serving Children and Youth with Special Health Care Needs.
  • Stay informed about available community resources and emerging programs and services.
  • Promote collaboration among agencies to strengthen the network of support available to children and families.

Qualifications

Candidates must meet one of the following licensure requirements:

  • Valid New Jersey Registered Nurse (RN) license; or
  • Valid New Jersey Certified Social Worker (CSW), Licensed Social Worker (LSW), or Licensed Clinical Social Worker (LCSW) license.

Additional qualifications include:

  • Bachelor's or Master's degree in Nursing (BSN/MSN) or Social Work (BSW/MSW).
  • Minimum of one year of pediatric case management experience, preferably working with Children and Youth with Special Health Care Needs.
  • Experience coordinating services across medical, developmental, educational, behavioral health, and/or social service systems preferred.
  • Strong communication, organization, advocacy, and interpersonal skills.
  • Ability to work effectively with children, families, professionals, and community partners.
  • Bilingual Spanish/English proficiency preferred.
  • Successful completion of an employer-paid physical examination, drug screening, and criminal background check.

The anticipated starting base pay for this position is:

  • $63,000 - $65,000 per year, depending on experience and qualifications.
