Job summary

Shriners Children's St. Louis is seeking a PRN Social Worker to join the Care Management team. The role involves assessments, planning, interventions and follow-up for patients and families across the continuum of care to ensure positive outcomes in a family-centered environment.

The successful candidate will hold an MSW, an active social work license in the practice state, and may pursue CM/ACM/ANCC certification. Compensation is determined by experience and department equity.

Qualifications

• Social Work Masters (MSW) required.
• SW License in State of Practice required.
• Case Management Certification (CM, ACM or ANCC) preferred.
• 3 Years of Social Work experience preferred.

Responsibilities

• Documents assessments, interventions, plans and evaluations in accordance with regulatory agency requirements and SHC policy.
• Coordinates the plan of care to achieve desired outcomes and integrates the work of the health care team by coordinating resources and services necessary to accomplish agreed-upon goals.
• Advocates by supporting or recommending services for the protection of the patient's health, safety and rights.
• Collaborates with patients, families, caregivers and the health care team locally and within the system to communicate, problem solve, and share accountability for optimal outcomes.
• Arranges services among community agencies, physicians, patients, family, caregivers and others involved in the plan of care.
