Job summary

Shriners Children's St. Louis is seeking a PRN Social Worker to join our Care Management team. The Social Worker provides assessments, planning, interventions and follow-up for patients and families across the continuum of care, contributing to positive outcomes in a family-centered environment.

Responsibilities include documenting assessments, coordinating plans of care with the health care team, advocating for patient health and safety, and arranging services with community resources.

Qualifications

• Must hold a Master of Social Work (MSW) and be licensed to practice Social Work in the state.
• Preferred: Case Management Certification (CM, ACM or ANCC) and 3 years of Social Work experience.
