Minimum qualifications include a BSW and MSW (or related field), with preferred licensure as an Oregon LCSW and experience in healthcare settings.

Qualifications

• Bachelor's degree in Social Work or related field with clinical experience in healthcare.

Responsibilities

• Screen and identify patients who need care management per high-risk criteria.
• Assess, develop, implement and monitor a discharge plan of care with an interdisciplinary team.
• Coordinate and facilitate discharge planning, involving patient/family.
• Provide psychosocial support and resources for post-discharge care.
