Job summary

Qualifications

• Current State license as a Registered Nurse.
• ACLS and BLS certification.

Responsibilities

• Coordinate patient care from hospital discharge through follow-ups.
• Provide education and instructions for procedures and testing ordered by providers.
• Review patient Health Risk Assessments and monitor charts for test results.
• Review and manage preoperative surgical clearance requests with providers as needed.
• Triage patient calls and provide health advice and referrals as appropriate.
