Job summary

ChildNet, Inc. in West Palm Beach, FL, is seeking a Nurse Coordinator to oversee care for medically complex children in partnership with the medical coordination unit, case managers, foster parents, and care providers.

The role requires two years of nursing experience and an Associate Degree in Nursing; a Bachelor’s Degree is preferred. This is a full-time, on-site position within a not-for-profit CBC agency serving Broward and Palm Beach counties.

Qualifications

• Two (2) years of nursing experience preferred.
• Associates Degree in Nursing required.
• Bachelor’s Degree preferred.

Responsibilities

• Screen medically complex children upon intake to guide placement.
• Assist caregivers in identifying medical specialists, scheduling appointments, and following physician recommendations.
• Provide medical care training and support to foster parents, including obtaining medical equipment and supplies.
• Act as a liaison between medical professionals and caregivers.
• Audit case files quarterly to ensure access to necessary medical care and complete documentation.
• Submit referrals to CMS for appropriate services.
• Maintain accurate records of all children with a medical diagnosis.
• Performs other duties as assigned.
