Job summary

Intermountain Health's Primary Children’s Outpatient Clinic in Salt Lake City is seeking a Social Determinants of Health Navigator to assess families for social drivers of health and connect them with community resources during clinic visits and by phone.

This on-site role collaborates with care managers and the social work team to support patient-centered, coordinated care and timely referrals.

Qualifications

• Demonstrated healthcare experience in a clinic or hospital setting.
• Demonstrated customer service with a focus on communications and problem resolution.
• Proficiency in advanced computer skills.
• Caregivers with home/community visits must maintain BLS cert, driver’s license, auto insurance, clean driving record and reliable transportation.

Responsibilities

• Liaise between department and external organizations including payers, physicians and patients for service authorizations.
• Monitor referrals and maintain communication with care managers, utilization review RNs, revenue cycle and payers.
• Retrieve medical records from Health Information Management for utilization review or QA.
• Deliver regulatory notices to patients within required timeframes with proper documentation.
• Perform clerical tasks to support care management, scheduling, and document handling.
• Assist with transition planning and coordinate with patients, families and providers.
