Job summary

Intermountain Health is seeking a Social Services/Utilization liaison for the Primary Children’s Outpatient Specialty Team in Salt Lake City. The role focuses on assessing families for social drivers of health needs and connecting them with resources during clinic visits and by phone.

The position is full-time and on-site, with responsibilities spanning referrals, record retrieval, regulatory notices, and collaboration with care managers to ensure holistic pediatric care.

Qualifications

• Healthcare experience in clinic or hospital settings.
• Customer service focus with strong communication and problem-solving skills.
• Proficiency with computers and software applications.
• Caregivers must maintain current BLS certification, driver's license, auto insurance, reliable transportation.

Responsibilities

• Liaises between department and external organizations (payers, physicians, patients, etc.).
• Transmits clinical information for service authorizations per contractual requirements.
• Monitors referral status and maintains ongoing communication.
• Requests and retrieves medical records for utilization review/QA.
• Delivers regulatory notices to patients within required timeframes.
• Performs clerical tasks to support care management operations (reports, scheduling, messages).
• Supports advance care planning and notarizes documents on request.
• Assists with patient choice processes and distributes provider lists.
• Aids transition planning by preparing transfer packets and coordinating with care teams.
• Collaborates with care managers to coordinate care and address social determinants of health.
