Qualifications

• Master's Degree in Social Work from an accredited institution.
• Current State Licensure as a Clinical Social Worker (LCSW) or Certified Social Worker, (CSW).
• Computer skills (email, word processing, and spreadsheets).

Responsibilities

• Organizes and prioritizes daily work by screening and assessing patient needs in area(s) of responsibility.
• Completes documentation as required and conducts psychosocial assessments.
• Educates, communicates, and coordinates with multiple patient care teams and outside agencies to ensure quality care.
