Job summary

Connecticut Children’s Center for Care Coordination seeks a skilled care coordinator to link children and families with medical, developmental, behavioral, educational and social services. You will work with families and health teams to implement a shared care plan and advocate for services across settings.

Applicants should have a Bachelor’s degree in a related field, at least 3 years of experience with families, and a CT driver’s license; bilingual Spanish/English is preferred.

Qualifications

• Bachelor's degree in social work, Family and Human Development, Public Health, or related fields.
• Minimum of 3 years’ experience working with families and community systems.
• Valid CT Driver’s License and willingness to drive throughout CT.

Responsibilities

• Provide comprehensive care coordination services linking families to medical, developmental, behavioral, educational and social services.
• Collaborate with families and care teams to develop and implement a shared care plan.
• Coordinate transitions across inpatient, outpatient, and behavioral health services to minimize gaps in care.
• Engage in grant-related activities, meetings, and community collaborations.
