Job summary

The University of Iowa Health Care — Care Coordination Division, Pediatrics Team — seeks a Social Worker III to provide direct social work services including assessment, treatment, interventions, and recommendations to facilitate movement through the continuum of care for patients and families.

The role develops and implements care plans, collaborates with an interdisciplinary team, coordinates discharge planning, and advocates for services, resources, and support throughout the patient’s stay

Qualifications

• Master’s degree with a major concentration in Social Work.
• 6 months -1 year of clinical social work experience.
• Excellent written and verbal communication skills.

Responsibilities

• Provide direct social work services including assessment, treatment, interventions, and recommendations to facilitate movement through the continuum of care for patients and families.
• Develop and implement plans of care responsive to patient/family concerns, health care status, available resources, and age-specific needs.
• Coordinate discharge planning and arrange community services to meet post-discharge needs.
• Collaborate with the interdisciplinary health care team and advocate for patients and families.
• Utilize social work knowledge to address team-related issues and support care transitions.
