Job summary

The University of Iowa Healthcare's Care Coordination Division seeks a Social Worker III to provide direct social work services across the continuum of care for patients and families. Responsibilities include assessments, treatment, and coordination with an interdisciplinary team to support post-discharge plans and community resources.

The role requires a Master’s in Social Work, Iowa licensure, and 6–12 months of clinical experience, with 100% effort on site in North Liberty, IA.

Qualifications

• Master's degree in Social Work required.
• Current, unencumbered social work license in Iowa.
• 6 months to 1 year of clinical social work experience.
• Excellent written and verbal communication skills.

Responsibilities

• Develops and implements care plans responsive to patient/family needs and clinical impressions.
• Identifies patients needing social work intervention.
• Performs social work assessments with patient and/or family.
• Provides range of social work services to address psychosocial functioning and post-hospital planning.
• Provides information and support to interdisciplinary team members.
• Facilitates patient/family groups when appropriate.
