Job summary

University of Iowa Health Care Department of Nursing is seeking a Nurse Clinician-Discharge Coordinator (PICU) to support the Nursing team with patient education and discharge planning under the supervision of the Director of Pediatric Care Coordination Division.

The role centers on clinical expertise and care coordination for pediatric patients with complex needs, including IMCU, and aims to optimize resources, promote throughput, and ensure smooth transitions to the next level of care.

Qualifications

• Baccalaureate degree in Nursing required.
• Pediatric ICU nursing experience within last 1–3 years preferred.
• Excellent written and verbal communication skills required.
• Demonstrated organizational, problem solving and critical thinking abilities.
• Current license to practice nursing required.

Responsibilities

• Discharge process management: ensure discharge orders are complete, review discharge summaries, verify AVS for appropriate HCI, provide teach-back with patients/families, monitor prescription fill time, assist with supplies, schedule follow-up appointments and confirm accuracy on AVS, provide clinic and emergency contact information on discharge.
• Lead daily unit bed huddles with the multidisciplinary team to manage care and review discharge plans.
• Collaborate with unit social work, patients and families to discuss discharge overview, equipment needs and education.
• Collaborate with multidisciplinary team to facilitate timely discharge and address barriers to shorten length of stay.
