Children’s Healthcare of Atlanta is seeking a seasoned nurse care coordinator to provide health service coordination for pediatric patients and families. The role requires 5 years of clinical experience, a Georgia RN license, and a BSN; graduate degree (MSN or MPH) is preferred.

Qualifications

• Experience with care coordination and case management recommended.

Responsabilités

• Works to ensure that assigned children from birth to age 18 have access to and participate in a medical home.
• Provides support to parents of newborns as needed and assists with continuation of care within assigned facility.
• Acts as primary point of contact between children and families and health providers as needed.
• Coordinates cares for chronically ill children.
• Coordinates joint meetings between family, primary care physician, facility staff and other providers as necessary.
• Serves as link between primary care physician, specialty providers, and other ancillary providers to ensure service delivery for assigned children.
• Maintains databases/logs and provides routine reports.
