Job summary

Children’s Healthcare of Atlanta seeks an experienced nurse to coordinate care for pediatric patients from birth to 18. You will liaison with families, primary care physicians, and specialists to ensure timely access to immunizations, screenings, and referrals.

Requirements include 5 years of clinical experience and GA RN licensure; BSN preferred with possible MSN/MPH. Regular daytime shifts with some weeknights or weekends may be needed.

Qualifications

• 5 years of clinical experience required.
• Experience in care coordination and case management preferred.
• Registered Nurse licensure in Georgia or ENLC/Compact.

Responsibilities

• Ensure children from birth to age 18 have access to a medical home.
• Support newborn parents and continuation of care within the facility.
• Serve as contact point between families, PCPs and other providers.
• Ensure immunizations per schedules and refer for additional services as needed.
• Coordinate care for chronically ill children and schedule joint meetings.
• Maintain databases and generate routine reports.
• Document follow-up coordination in patient records.
