Job summary

Children’s Wisconsin is seeking an Integrated Case Manager to coordinate transitional care for pediatric patients as part of a dedicated medical team. The ICM collaborates with providers to ensure safe and efficient discharge planning and may perform Hospital Case Manager duties.

Education includesRN licensure in Wisconsin, with 2+ years of case management experience preferred. Pediatric discharge planning, home care experience valued.

Qualifications

• RN license required (Wisconsin)
• 2+ years of case management experience preferred
• Pediatrics, discharge planning, or home care experience preferred

Responsibilities

• Initial and ongoing assessment of patient clinical status and care coordination plan.
• Lead and manage care coordination across providers, family/caregivers, and facilities.
• Attend rounds with Provider teams and document admissions, length of stay concerns, and discharge planning.
