Requirements include an Associate's Degree in Nursing, a current RN license, and 1+ year hospital/med/surgical experience with strong communication and organizational skills.

Qualifications

• Associate's Degree in Nursing is required.
• Current, unrestricted RN license in the state of employment.
• 1+ years of experience with/knowledge of community resources, policies, and procedures.
• 1+ years of hospital and inpatient medical/surgical unit experience.
• 1+ years of clinical experience with/knowledge of age groups including unborn to geriatric.

Responsibilities

• Directs disposition of patients and utilization considerations.
• Facilitates interdisciplinary collaboration.
• Communicates with staff, physicians, patients, and families.
• Coordinates care processes to achieve optimal outcomes.
• Assists with discharge planning and post-hospital services.
• Documents in medical records and coordinates interventions.
