Job summary

VCU Health is seeking a Nurse Patient Navigator to advocate, educate, and guide patients and families. You will coordinate care with a multidisciplinary team and serve as the single contact point for assigned patients and caregivers.

In this role, you will apply nursing knowledge to support treatment plans, provide counseling, and ensure access to comprehensive services. Weekday schedule with potential evening/weekend coverage may apply across VCUHS locations.

Qualifications

• RN licensure in Virginia or eligible/compact state
• Minimum 3 years of nursing, patient education, case management or navigation
• Specialty nursing certification in Nurse Navigation or area of specialty within 1 year of hire

Responsibilities

• Advocate, interpret, educate, and counsel a group of patients as Nurse Patient Navigator
• Coordinate care with a multidisciplinary team to ensure high-quality, comprehensive services
• Serve as the single point of contact to assigned patients and caregivers
• Provide ongoing support to patients and families throughout treatment
