Job summary

VCU Health in Richmond, VA is seeking a Nurse Patient Navigator to advocate for patients, coordinate care with a multidisciplinary team, and support patients and families throughout the treatment process.

The role requires RN licensure in Virginia, a BSN, and at least 3 years of nursing experience with patient education and case management. Master’s degree and nurse navigation leadership skills are preferred.

Qualifications

• Current RN licensure in Virginia or eligible or compact state.
• BSN from an accredited school.
• Minimum of three (3) years of nursing, patient education; case management and/or navigation.
• Preferred: nurse navigation or leadership experience.

Responsibilities

• Advocates for patients and families and serves as a single point of contact.
• Collaborates with a multidisciplinary team to coordinate care and services.
• Utilizes nursing knowledge to support patients throughout treatment.
• Follows physician/licensed independent provider orders and ANA ethics guidance.
• May require coordinating across VCUHS locations and clinical settings.
