Job summary

Qualifications

• RN licensed in Virginia or eligible, including compact state licensure.
• Minimum of three years in nursing with patient education, case management or navigation.
• Bachelor’s degree in nursing from an accredited program; a Master’s degree preferred.

Responsibilities

• Manage care for assigned patients with a multidisciplinary team to ensure quality care.
• Serve as the single point of contact for patients and their caregivers.
• Provide education, counseling and advocacy throughout treatment.
• Support patients/families, coordinating services across settings as needed.
