Job summary

Connecticut Children’s in Hartford seeks a licensed clinical social worker (LCSW) to identify psychosocial and psychiatric needs of patients and families through clinical assessment, crisis intervention, and supportive counseling in collaboration with the health care team.

You will work with the Emergency Department behavioral health team, coordinate resources, advocate for patients, and help with intake, referrals, and transitions of care, while upholding the hospital’s mission and values.

Qualifications

• Master's in social work required.
• Minimum 2 years related experience serving the needs of children and families.
• LCSW certification required.
• Psychiatric assessment, triage and intake experience preferred.
• Nonviolent Crisis Intervention training recommended within one year.

Responsibilities

• Assess and plan patient care with psychosocial and psychiatric assessments of patients and families.
• Develop therapeutic plan and provide crisis intervention.
• Coordinate care with multidisciplinary team; liaise with payors; ensure transitions across levels of care.
• Oversee intake and referrals; maintain contracts and relationships.
• Participate in performance improvement and research; cost-effective utilization of resources.
• May provide medical on-call services as part of on-call team.
• Support mission and demonstrate cultural sensitivity and diversity.
• Demonstrates knowledge of age-related differences from neonate to young adult.
• May supervise students or contribute to licensure supervision.
