Job summary

Connecticut Children’s in Hartford, CT, seeks a social worker to meet psychosocial needs of patients and families through clinical assessment, crisis intervention, supportive counseling, and collaboration with the health care team. Reports to the Manager of Child & Family Support Services.

Education: Masters in Social Work; Experience: minimum 2 years serving children and families; Licensure: LMSW within one year of hire, LCSW preferred. Strong interpersonal and communication skills required.

Qualifications

• Masters in Social Work required.
• Experience with psychosocial needs of children and families preferred.
• Ability to engage with patients and families and collaborate with the care team.

Responsibilities

• Develop effective working relationships with patients and families to advocate and problem solve.
• Conduct psychosocial assessments including social, psychological, cultural and financial information.
• Utilize clinical judgment to determine level of support needed.
• Develop therapeutic plan based on psychosocial assessment.
• Evaluate progress and address termination and transitions with families.
• Connect families with hospital and community resources.
• Implement crisis intervention and therapy modalities as needed.
• Document resource needs and referrals in patient records.
• Serve as consultant to peers and interdisciplinary team.
