Job summary

Connecticut Children’s Medical Center seeks a Primary Care- Family Support Clinician (MSW) to address psychosocial needs of patients and families through assessments, crisis intervention and counseling. This on-site role reports to the Manager of Child & Family Support Services and collaborates with the healthcare team and community resources.

The LMSW may supervise students; LCSW is preferred. Qualifications include a Master of Social Work, 2+ years of relevant experience, and Crisis

Qualifications

• Education: Masters in Social Work required.
• Experience: Minimum of 2 years related experience serving the needs of children and families.
• LMSW required within one year of hire; LCSW preferred.
• Knowledge of family centered care principles and practices.
• Skills: Strong interpersonal and written communication skills; ability to adapt in a high-intensity environment.

Responsibilities

• Develop an effective working relationship with patients and families to advocate and problem solve in order to enhance their functioning.
• Conduct timely psychosocial assessments and evaluations of patients and families including social, psychological, cultural and financial information.
• Utilize clinical judgment to determine level of assistance and engage in complex problem solving.
• Develop therapeutic plan based on psychosocial assessment.
• Evaluate progress and address termination and transitional processes with patients and families.
• Connect patients and families with hospital and community resources to meet medical and psychosocial needs.
• Provide crisis intervention and/or individual, group, and family therapy as needed.
• Document in patient records regarding resource needs and referrals.
• Serve as a resource to peers and active member of interdisciplinary medical team.
